How to Register
If you are a prospective student interested in learning more about Disability Services at the Mount, you are welcome to peruse this page and contact us at any time.
If you are an incoming or current student, the first step in registering with Disability Services is to self-identify as a student with a disability and request accommodations. See the steps below to learn how to register and submit documentation to our office.
Step 1: Complete the Online Student Application+-
So that we may get to know you, please complete our Online Student Application.
Step 2: Submit Disability Documentation+-
After you submit your request form, you will be invited to upload any supporting disability-related documentation. If you are not able to upload documentation right away, you may provide it at a later date. Please note: your file will be held as “pending” until we receive your documentation.

- This form serves as a guide for medical or psychological professionals as to the information our office needs to complete your file. Your health provider may use this form or provide the same information on their own professional letterhead.

- Disability documentation will look different for each individual student. Please review our guidelines, and contact us at any time with questions.
Step 3: Schedule a Meeting+-
After we receive and review your documentation, a Disability Services staff member will contact you to schedule a meeting. This first meeting will be an interactive session designed to give our office a narrative of your disability and to determine your access and accommodation needs. We will review all accommodation policies and procedures as well as your student rights and responsibilities.
